You should bridge the connections for the choke, and reverse the dc in connector. (notice this is another connector, not the one stated in the BOM)

Notice the LM301 is soldered directly to the pcb. However you should use IC sockets.
There are 2 things to keep in mind:
The BC558B needs to be mounted backwards (flat side of the transistor faces round side of the pcb print):
You have to swap 2 pins of the 3958 - use insulation:
